All volunteers and staff of SVMHA must undertake a Child abuse registry check annually .

The process has been streamlined and there will be no cost to you.

1. Print the attached 3 page form
2. MAKE SURE YOUR PRINTER IS SET TO PRINT THE WHOLE PAGE the bottom signature block on page one must be intact
3. Page one:  Complete Section B  ---- Date and Sign  B-7
4. page two:  Date and Sign
5. page three: Print your name in the Subject's Name space
6. Scan the three Pages  ---- name it CAR Application (your last name)
7. Save it to your computer
8. Click on the Tab below these instructions to up load your Signed Application

The only step left is for me to verify your identification.  If I need to view your Identification I will contact you and invite you to do this via zoom.
